    Ontario Health Teams collaborate with community partners to implement Ontario Health's priorities in alignment with the Ministry of Health's directives. Their shared mission is to help local communities work together to make the healthcare system better, improve health for everyone, and create a better experience for patients and healthcare providers.

    The key principles of integrated care include focusing on the person, connecting services across organizations, using teams made up of different types of healthcare professionals (like doctors, nurses, social workers, and therapists) who work together, ensuring care is continuous, and using technology to help support care. The benefits of integrated care include improved experiences and outcomes for patients, clients, families and caregivers. It also supports healthcare providers, improves the health of the community, helps manage complex health conditions, and saves money for the health system.

  • Who is involved in the North Simcoe OHT?
    The North Simcoe OHT is made up of the following partners: Anchor Partners: NSOHT Anchor Partner organizations use a consensus decision-making approach to collaborate on OHT priorities and system improvements. Two co-chairs are elected from the Anchor Partner organizations to provide leadership to the NSOHT Collaborative Council and the OHT Director Collaboration Partners The North Simcoe OHT Collaboration Partners help advance OHT priorities and system improvements through their engagement at the OHT Collaborative Council and on related working groups and committees. Primary Care Network (PCN) PCN members play a key role in co-designing the OHT, building connections with community partners, and exploring new programs to enhance patient care and practice support in North Simcoe. Patient & Family Advisory Council (PFAC) Members engage in an advisory capacity and make recommendations to the North Simcoe OHT on matters that make an impact on providing the best patient, family, caregiver and resident experiences and outcomes. Learn More NSOHT Implementation Team Under the leadership of the NSOHT Director, the implementation team is responsible for operationalizing and implementing the North Simcoe OHT strategic plan and supporting the OHT partners as they work towards the OHT priorities areas. Working Groups They are responsible for planning, implementing, and evaluating projects and initiatives and work collaboratively to address specific health priorities, improve patient outcomes, and streamline service delivery across the healthcare system.
