About

The North Simcoe Ontario Health Team (OHT) is a group of over 20 health, community and social service providers working together to provide people with better connected and more convenient care, closer to home. We collaborate with patients, families, caregivers, and the community to co-design the best possible care, improving the healthcare journey for people and providers in North Simcoe.

What Is the Role of an OHT?

Ontario Health Teams collaborate with community partners to implement Ontario Health's priorities in alignment with the Ministry of Health's directives. Their shared mission is to help local communities work together to make the healthcare system better, improve health for everyone, and create a better experience for patients and healthcare providers.

Our Strategic Priorities

Our five key priorities are:
 

  • Improving access to quality care by providing accessible, streamlined pathways.

  • Ensuring the community’s voice is heard and valued in shaping our initiatives.

  • Advancing a health system that provides equitable healthcare access, quality, and outcomes for all individuals and communities we serve.

  • Leveraging digital advancements to improve healthcare accessibility, efficiency, and effectiveness for both patients and providers.

  • Strengthening our leadership capacity to drive collaborative success.

Our Geographical Area

The North Simcoe OHT is found on the traditional territories of the Anishinaabek Nation and is home to the Chippewa TriCouncil First Nations comprised of the Beausoleil First Nation, the Chippewas of Rama, and the Chippewas of Georgina Island. This area is also home to the Moose Deer Point First Nation, the Wahta Mohawks, the Georgian Bay, and Moon River Métis Councils.

 

The North Simcoe OHT supports the communities of Midland, Penetanguishene, Tiny Township, Tay Township and Christian Island.
